Welcome
I am the school nurse for Chocowinity Primary School. I started school nursing in 2011 with Beaufort County Schools and obtained my National School Nurse Certification in 2015. I have ten years of acute care nursing experience with a background in medical, surgical, pediatric and intensive care nursing. My goal is to help Chocowinity Primary School provide a healthy and safe learning environment for all students and staff.

Diet Order
This form to be completed by the student’s medical provider and parent for a Diet Order to be followed at school. Diet orders are for severe allergies and medical necessity. Please reach out to the school nurse if your diet concern is not a severe allergy or a medical necessity.

Medication Form
This is the form required for medications to be administered at school. It is to be completed by the medical provider and parent prior to bringing medication to campus.

NC Health Assessment Transmittal Form
This form is to be completed on entry to North Carolina public schools. This includes Preschool, Kindergarten and new to the NC public school system.
